When you come in for your appointment, if you haven’t already enrolled, we will discuss joining our patient portal, which we strongly encourage you to use to communicate with our office.

How do I request medication refills?
When you come to Capital Region Family Medicine for your visit, we will discuss if you require any medication refills. In between visits, the best way to request prescription refills is through our patient portal, FollowMyHealth. To request a refill, log into your account, select the “My Health” tab, and then “Medications” from the menu. You can request a refill by selecting the pill bottle icon next to the medication you require. Please allow 48 business hours for us to process your request.

For your convenience, our practice and Community Care Physicians offer an online medical records request form to send records outside of our practice. Please request records on our website to request records from us. Please note that different forms are used if you are the patient or if you are a patient representative making the request. All requests will be processed within five (5) to ten (10) business days of receipt of the requests.
How do I request an appointment?
If you are new to our practice, please call our office at 518-980-9040 to schedule an appointment. You can also request an appointment online using our become our patient form on our website.
If you are already a patient of our practice, you can call us to schedule an appointment, or if you have a patient portal account, request an appointment through the portal by selecting “Appointments” on the desktop or mobile application.
What do I do if I have an urgent need or problem after hours?
Capital Region Family Medicine has a physician on call after business hours from 3pm to 7am, until our office reopens. If you have any medical need after hours, as long as it’s not life threatening, call our office first at 518-980-9040. We can resolve your problem or to direct you to the proper level of care. You may not need to visit an emergency room or urgent care.
If you do require urgent care services, we recommend using CCP’s Urgent Care Latham at 6 Wellness Way, Latham, in the same building as our office. Urgent Care Latham has a dedicated entrance directly off the main parking lot of Wellness Way and they share a medical record with our office for continuity of care.
Who do I contact for a billing question or issue?
We have an online billing platform to pay bills, etc, accessible on our website. For billing questions, please visit the Pay My Bill menu on the top of our website, or call CCP’s customer service team at 518-782-3700.
If you have a bill with Community Care Physicians and a cell phone number/email address on file with us, you will receive a text/email message with a link to view your statement and make a payment. Your account number is automatically populated when you click the link from the text or email. If you don’t have a text or email, you can always visit our website and select the “Pay My Bill” link at the top of the page to access our online billing portal at any time, 24/7. You’ll need your account number from your statement and your date of birth to log in through the Community Care website.
